To request a fair hearing, you may fax, write, or call the Office of Temporary and Disability Assistance (OTDA), Office of Administrative Hearings at:

In New York City, you may also request a fair hearing in person at either of the two OTDA Office of Administrative Hearings located at:

There are time limits on requesting a fair hearing, so it is usually best to request a fair hearing as soon as possible.  Keep a copy of your request for your records.  Once the fair hearing request is processed, you should receive an acknowledgment form and a scheduling notice from the State.

PLEAS NOTE :  Fair hearing requests must go to the State OTDA, not to your local Job Center, Food Stamp Center, Medicaid Office, local legal services office, or to Project FAIR.  If you do not receive an acknowledgement form confirming your fair hearing request within a few weeks, you may want to call OTDA to follow up with your request.
