Reasons You Might Not Have Received Your Mail

Often times, benefits recipients or applicants claim they did not receive a notice from the City Agency/HRA/Job Center prior to an action being taken on their case. This handout provides you with reasons as to why you might not have received this mail. Some of them may apply to you.

Physician’s Employability Report

If you are claiming a medical exemption or a change from your assigned work requirement, have your treating physician fill this report out as thoroughly as possible and submit it at your Fair Hearing as documentation of your physical condition and limitations. Keep a copy.

Pro Se Evidence Packet Request (Feb 2013)

It is good practice to always request a copy of the evidence packet for your Fair Hearing once you have been given a Fair Hearing number. Use, this form, with the instructions, to request a copy of your evidence package, the packet of documents the agency is going to submit to the Judge the day of your hearing, as soon as you get a Fair Hearing number. Keep a copy of your fax receipt or date/time stamped copy if you deliver it in person, and take it with you to your Fair Hearing.
